Not always. One variable-speed furnace and AC with zone dampers and two or three thermostats can serve many large homes, as long as the ductwork is sized for it. Houses with a long single-story wing or a very large lower level sometimes do better with a second system. A room-by-room Manual J load calculation shows which way to go before you spend the money.

Noise usually means high static pressure: too few returns or undersized ducts forcing the blower to push hard. Big two-story rooms make it worse because the grilles sit where you hear them. Adding return capacity, using larger grilles, or moving to a variable-speed blower that spends most of its time at low speed all help. We measure static pressure before recommending any of it.

Usually it's clogged or out of balance. HRVs and ERVs in newer homes need their filters and core cleaned, and the intake and exhaust airflows should roughly match. If the outdoor hood is blocked by frost, lint or a nest, the unit can run without moving much air. We clean it, check the airflows and set the controls so it runs on a schedule that suits the season.

Equipment from that era is reaching the age when major parts start to fail: inducer motors, control boards and blower motors on furnaces, compressors and coils on air conditioners. A yearly tune-up catches the smaller items.
